Job summary

Rock County seeks a qualified clinician to deliver comprehensive Assertive Community Treatment for individuals with severe mental illness through the CSP program. You will manage treatment plans, coordinate services, and collaborate with a multidisciplinary team to support recovery and independence.

Role requires a Master’s in Social Work and a Wisconsin LCSW/LPC license within 3 years, plus experience with Medicaid/Medicare regimes and consumer-centered care.

Qualifications

• Requires a Master’s Degree in Social Work and an active WI license (LCSW/LPC) within 3 years of hire.
• Experience with assertive community treatment and DHS 63 regulations is preferred.

Responsibilities

• Provide comprehensive ACT services including case management, symptom management, supportive psychotherapy and psychosocial rehab.
• Develop and maintain client records, benefits, plans and reports per DHS 63 and Medicaid/Medicare requirements.
• Collaborate with a treatment team, families and collateral agencies to coordinate care.
• Advocate for least restrictive living arrangements and, when needed, testify at court proceedings.
