Job summary

CareSource seeks a Community Based Care Manager to collaborate with an interdisciplinary team and community partners to coordinate holistic care for members. You will lead assessments, develop person-centered care plans, and ensure timely access to needed services while addressing social determinants of health.

In this role, you will facilitate ICT meetings, engage with members in diverse settings, and monitor progress to improve outcomes.

Qualifications

• Nursing degree or health care bachelor’s degree required
• RN/Social Worker/Clinical Counselor licensure required
• Minimum 3 years nursing, social work, counseling, or health care experience required
• Medicaid/Medicare managed care experience preferred

Responsibilities

• Engage the member and natural supports through assessments using a trauma-informed approach
• Facilitate regular inter-disciplinary care team (ICT) meetings
• Develop and update person-centered ICP with ICT
• Identify and manage barriers to care plan goals
• Coordinate with providers for post-discharge planning and transitions
• Document care coordination activities in the member record
• Educate member and natural supports about treatment options and community resources
• Monitor member progress and adjust interventions per guidelines
