Job summary

CareSource is seeking a Community Based Care Manager to work with an interdisciplinary care team, providers and community organizations to coordinate services and create person-centered care plans. You will assess and address behavioral, physical, and social determinants to improve member health outcomes.

Responsibilities include engaging with members and natural supports, leading ICT meetings, educating on options and resources, and ensuring timely post-discharge planning.

Qualifications

• Nursing degree or Bachelor’s in health care; relevant work experience accepted.
• Licensure as RN, Clinical Counselor or Social Worker required; advanced licensure preferred.
• Minimum 3 years in nursing, social work, counseling or related health care fields; Medicaid/Medicare exp. preferred.

Responsibilities

• Engage with member and natural supports to assess needs and develop ICPs.
• Coordinate ICT meetings and facilitate communication among providers.
• Educate members about treatment options, resources, benefits and services.
• Monitor member progress, evaluate ICP effectiveness and adjust care plans.
• Travel to member, provider and community sites as needed; ensure timely care.
