Job summary

CareSource is seeking a Team Lead, Community Based Care Management to provide front line management and support to departmental leadership in developing and executing the Care Model in alignment with corporate goals.

The role involves supervising staff, recruiting and training, conducting training meetings, and using data to monitor service quality. Travel may be required as part of a mobile position in care delivery settings.

Qualifications

• Requires RN degree program completion, or Social Work/Master's licensure eligibility.
• Minimum 3 years of healthcare experience.
• Minimum 2 years of managed care/case management experience; supervisory experience preferred.

Responsibilities

• Lead and supervise staff to meet departmental expectations.
• Recruit, hire, onboard, train and support team.
• Train and lead staff in CM program administration through meetings.
• Prepare and audit reports; represent unit in meetings.
• Ensure staffing policies, SOPs are current and compliant.
• Support Managers with new projects/processes.
• Oversee system documentation per guidelines.
• Provide technical/content support for strategic alignment.
• Collaborate with internal resources to meet service levels.
