Qualifications

• RN, SW, or Clinical Counselor licensure required.
• 3+ years of nursing, social work, counseling, or health care experience required.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage member and natural supports using strength-based assessments.
• Lead ICT meetings to plan and review care.
• Develop and update ICP in collaboration with ICT.
• Identify and address care barriers and implement evidence-based interventions.
• Coordinate with providers and community resources to ensure seamless care.
