Job summary

Pathways Forward is seeking a Service Access Coordinator to oversee intake, eligibility verification, registration, and waiver administration for children and adults with ID/ASD. In-person, full-time role working 8:30 a.m.–5:00 p.m. weekdays.

The ideal candidate will have a bachelor’s in a human services field with relevant experience, or a master’s with fewer years, and may require bilingual abilities. A PA driver’s license and clearances are necessary.

Qualifications

• Bachelor’s degree in a human services field with 2+ years of relevant experience or Master’s with 1 year experience, or Associate’s with 4 years experience.

Responsibilities

• Provide outreach to the community and engage with individuals, families, and local entities.
