JOB PURPOSE: Provides service coordination foradults and children with intellectual and developmental disabilities, autism,and pervasive developmental disorder to assist them in gaining access tomedical, social, educational, vocational, and other needed services. Hasresponsibility for maintaining theGeneral Revenue Internal Interest List (GRILL) by calling to explain availableservices, initial, assessments, discovery process, completing enrollments, and coordinationof the Person Directed Plan and desired outcomes.

EDUCATION AND EXPERIENCE REQUIRED: A bachelor's or advanced degree from an accreditedcollege or university. Case Management or Service Coordination experiencerequired.

DRIVING REQUIRED: Yes EEOCCODE: Professional

KNOWLEDGE, SKILLS & ABILITIES:

• Abilityto compose documents according to departmental and state standards.
• Computer-skilledin the use of technology with Microsoft Office and all Googleapplications.
• Abilityto input data into computer, make calculations, complete paperwork,produce reports.
• Familiaritywith community resources.
• Abilityto build rapport with individuals and support systems.
• Must bephysically able to conduct personal/business visits to include walking onuneven surfaces, climbing stairs, and standing for periods of time ifrequired (with or without a reasonable accommodation).

• Liabilityinsurance required if employee will operate personal vehicle on Centerproperty or for Center business. Must be insurable by Center'sliability carrier if employee operates a Center vehicle or drives personalcar on Center business.
• Travelmay be required within the 4 county service area.
• Mustcomplete all training requirements in month in which they are due.
• Availabilityof a personal vehicle for business use will be required.
• May berequired to work hours outside of the M-F 8-5 framework.
• Workmay take place within office setting, community setting, nursingfacilities and within the residences of individuals.
• Must complete a computer skills test.
• Mustsubmit to and pass a pre-employment drug test.
• Mustsuccessfully pass all background screens as required by the state.
