Job summary

Guilford County Government seeks a Continuum of Care Specialist to support homeless prevention, diversion, and outreach initiatives. You will help plan meetings, analyze performance data, and participate in CoC activities throughout the year.

The role emphasizes program performance monitoring, technical support, and system-wide improvements in alignment with HUD requirements. Collaboration with Planning staff and partners is essential.

Qualifications

• Master’s degree in Social Work or related field.
• Demonstrated knowledge of homelessness and CoC practices.
• Experience with HUD policies and program requirements.
• Ability to analyze performance data and prepare reports.
• Ability to work with diverse stakeholders and maintain discretion.

Responsibilities

• Monitor and evaluate CoC- and ESG-funded programs for performance.
• Lead APR reviews, quality assurance, and HUD submission readiness.
• Analyze System Performance Measures and identify improvement opportunities.
• Provide technical assistance and training to providers.
• Coordinate communications and support HMIS data quality efforts.
