Qualifications

• High school diploma or equivalent.
• Current PA CNA license with hospice/home health experience.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provides personal care services to patients.
• Assists with ambulation, transfers, ROM, positioning, and household tasks.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and environmental tasks.
• Observes, reports and documents changes in patient status to the primary nurse.
• Follows written instructions for personal care prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests.
• Other duties as assigned.
