Job summary

StoneBridge Senior Living is seeking a compassionate Certified Nursing Assistant to support nurses and care teams in providing attentive, high-quality care to residents. The role emphasizes assisting with daily living activities, mobility, nutrition, documentation, and respectful communication.

We value patience, empathy, and teamwork in a facility dedicated to Rehabilitation, Assisted Living, Memory Care and Skilled Nursing.

Qualifications

Certified Nursing Aide certification required (in good standing)
High School Diploma or GED preferred
Current CPR/BLS certification preferred
At least one year of CNA experience in long-term care preferred
Ability to communicate effectively in an interdisciplinary team

Responsibilities

Provides supportive services to nursing staff and other staff as needed.
Assists residents with activities of daily living per care plans and policies.
Assists residents with lifting, turning, moving, positioning, and transporting.
Coordinates dining room services, meal setup, delivery, and feeding assistance; documents intake.
Delivers nutritional supplements and records intake.
Assists with weighing residents and reports changes to nurse.
Monitors skin condition and reports issues to nurse/supervisor.
Assists with toileting program activities and documents progress.
Keeps supervisor informed of factors affecting performance.
