Job summary

StoneBridge Senior Living is seeking a compassionate Certified Nursing Assistant to support our nursing team from 7 PM to 7 AM on weekdays, with weekend coverage as needed. You will assist residents with daily living activities and carry out care plans with empathy.

Qualifications

• Certified Nursing Assistant in good standing or graduate of state-approved CNA program and passed competency exam.
• High School Diploma or GED preferred.
• Current CPR/BLS certification preferred.
• At least one year of experience as a CNA in long-term care preferred.
• Able to communicate effectively in written and verbal form in an interdisciplinary care setting.

Responsibilities

• Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
• Assists resident with activities of daily living per care plans and policies.
• Assists with lifting, turning, moving, positioning, and transporting residents.
• Coordinates dining room services during meals, including setup, delivery, feeding, and documentation.
• Delivers nutritional supplements and documents intake.
• Assists with weighing residents and reports changes to nurse/supervisor.
• Monitors skin condition and reports issues to nurse/supervisor.
• Assists with toileting program activities.
• Completes flow sheets and documents progress daily.
• Keeps nurse/supervisor informed of factors interfering with duties.
• Reports changes in resident condition to nurse and supervisor.
