The role focuses on partnering with families of children with disabilities to access services, supports, and resources to help them thrive at home and in the community. You’ll work to identify strengths and needs, coordinate individualized plans, and advocate for youth while collaborating with schools, providers, and county programs.

• Knowledge of or experience with children, youth, or young adults with disabilities.

Responsibilities

• Build trusted relationships with children, youth, families, and supports.
• Coordinate services addressing developmental, emotional, physical needs.
• Develop family-centered service plans based on each youth's needs.
• Connect families with formal and informal supports and resources.
• Advocate for youth and families to access available services.
• Promote inclusion and safety in home and community.
• Document accurately and maintain program requirements.
