Job summary

Rock County is seeking a CLTS Program Specialist to manage intake, assess eligibility for children, and coordinate services across multiple systems. You will document in Avatar and collaborate with case managers, SSCs, and community partners to ensure timely access to supports.

The role emphasizes crisis response, risk assessment, and ongoing follow-up with families, within a county-wide service framework. A valid driver’s license is required.

Qualifications

• Bachelor's degree in counseling, social work, psychology, or human services.
• Social Work Certification preferred.
• Minimum 2 years of CLTS experience and in good standing with the agency.
• Certified in the Children's Long Term Care Functional Screen.
• Must possess and maintain a valid driver's license and auto insurance.

Responsibilities

• Conduct functional screens and assessments for children.
• Complete/Update Functional Screens; document in Avatar and case files.
• Provide advocacy and coordinate with partners for safe, holistic care.
• Respond to crisis needs and manage safety concerns for families.
