Job summary

Wood County Human Services Department is seeking a CLTS Support & Service Coordinator I/II to assess, plan and coordinate needed support for children with disabilities and their families. The role emphasizes service delivery, intake, and collaboration with schools and healthcare providers.

Responsibilities

  • Assess, plan and coordinate needed support and services for children with disabilities (CCOP/CLTS).

GENERAL FUNCTION

Assess, plan and coordinate needed support and services to children who have physical, cognitive, and emotional disabilities. Service Coordinators also support the immediate family members of these children and help problem solve issues/concerns that arise. This position has a significant impact on the children and families served, as well as on the organization, as it allows Wood County children to receive the items and services they are in need of, to keep them safe and allow them to stay within their home and community, (least restrictive and most natural environment) rather than having them placed in more restrictive and less natural environments.

RESPONSIBILITIES
ESSENTIAL FUNCTIONS
  • Conduct assessments and develop and implement Individual Service Plans for children in the Children’s Community Options Program (CCOP) and the Children’s Long-Term Support (CLTS) Medicaid Waivers Program.
  • Provide direct supportive services to children and families on caseload including information/referral, supportive counseling, assistance in making application for financial assistance programs, etc.
  • Conduct home visits to discuss family issues/concerns and document all relevant information within CCOP and CLTS Medicaid Waivers Program.
  • Complete Wisconsin Children’s Long Term Support Functional Screens (initial, annual, updates).
  • Complete paperwork other than Functional Screens (case notes, letters, emails, faxes, funding requests/authorizations, Gainwell Technologies/Forward Health Authorizations authorizations, Individual Service Plans (ISP), Outcomes, referrals, reading over medical records, reviewing Individual Education Plans (IEP), reviewing parents/guardians requests of items/services, completing incident reports through CITR, entering data into the Forward Health Portal using Eligibility and Enrollment Streamlining (EES), high cost funding requests, parental fee calculations, other state and county forms, assessments, etc.).
  • Make home visits to conduct intakes, reviews, check-ins, provide support, attend school meetings and/or medical appointments, etc.
  • Make telephone calls to clients, providers, state contacts, therapists, evaluators, contractors, and other professionals and non-professionals associated with the provisions of the County operated services.
  • Create and help manage CLTS Waiver Third Party Administration TPA authorizations with the Business office staff.
  • Perform all duties in compliance with the Medicaid Home and Community-Based Services (HCBS) Waiver Manual for the CLTS Waiver Program and standards set out in Medicaid Targeted Case Management regulations.
  • Manage the expenditures of CCOP funds, submit timely vouchers, and work closely with the Business office staff in completing State required Reporting Systems (Human Services Reporting System and Program Participation System).
  • Responsible to know and practice the safety policies of Wood County. Perform all job tasks in a safe and prescribed manner.
  • Must be familiar with and exemplify the Wood County Core Values and serve with courtesy and respect in accordance with the Wood County Code of Conduct.
B. OTHER DUTIES
  1. Research items and services that clients are in need of.
  2. Be familiar with and follow established Wood County Human Services Department policies and procedures, as well as be compliant with federal and state policies and standards.
  3. Participate in unit meetings and attend training/conferences as required.
  4. Assure that proper case files are maintained in compliance with Department policy and any applicable regulations.
  5. Monitor service costs for clients in CLTS and CCOP.
  6. Any other duties as may be assigned.
Job Qualifications:
MINIMUM TRAINING AND EXPERIENCE TO PERFORM ESSENTIAL JOB FUNCTIONS:
CLTS Support & Service Coordinator I - Uncertified
  • 1. An associate’s degree in a human services-related field and at least two years of experience working with individuals and/or children with disabilities required. Must have knowledge working with children with DD (Developmental Disabilities), SED (Severe Emotional Disturbances), and PD (Physical Disabilities). In lieu of degree, any combination of training in a health or human services-related field and experience in long-term care case management and/or case management for individuals with disabilities or special needs that totals four years. Examples include but are not limited to: One year of coursework or training in a human services-related field and three years of experience in long-term care case management, or four years of experience in long-term support case management.
  • 2. A Bachelor of Arts/Bachelor of Science degree in a health or human services related field and at least one (1) year of professional experience working with individuals and/or children with disabilities required. Must have knowledge working with children with DD (Developmental Disabilities), SED (Severe Emotional Disturbances), and PD (Physical Disabilities).
All other duties apply to both Certified and Uncertified
  • 2. Must be knowledgeable of the service delivery system, the availability of integrated services and resources, or the need for such services and resources to be developed.
  • 3. Must complete the approved DHS’ Children’s Long-Term Support (CLTS) Program Training for Support and Services Coordinators (SSCs) and the Mandated Reporter Online Training. Until training is completed, the coordinator must work under the direct supervision of a qualified Support Service Coordinator and/or supervisor.
  • 4. Must become certified to perform the Children’s Long Term Support Functional Screen within one (1) month of employment.

Candidates must possess basic everyday living skills, the ability to understand and follow directions, the ability to read and write (for reports); and the ability to interview, counsel and communicate. This position requires travel to and from off-campus sites. Possession of a valid driver’s license, as well as a licensed and insured automobile is required, or other means of reliable transportation immediately available to individual.

Basic office equipment used: computer/tablet, printers, telephone, copy machine, fax machine and calculator is required, as is operational familiarity with Microsoft Word and Excel software and Streaming Healthcare Solutions software.
