Job summary

Outagamie County is seeking a Case Manager for the CLTS program to coordinate services for children and families, acting as a primary contact for clients and partner agencies.

Responsibilities include gathering information, assessing needs, linking to resources, making referrals, and documenting in accordance with state guidelines. The role requires a degree (RN/BA) with relevant experience and a valid driver's license.

Qualifications

• Bachelor's degree or RN license; Social Worker certification preferred.
• Two years of experience in health or human services.
• Valid driver's license.

Responsibilities

• Obtain information from clients and assess needs.
• Provide information meeting clients' goals with person-centered approach.
• Maintain knowledge of vocational, housing, and community resources.
• Make referrals to other agencies as needed.
• Maintain relationships with human services and medical agencies.
• Provide educational presentations and participate in community events.
• Identify system needs and participate in program development.
• Attend meetings and coordinate with teams.
• Maintain clinical files and prepare reports per MA guidelines.
• Receive and screen referrals for children with disabilities.
• Perform assessments to determine program eligibility.
• Provide ongoing case management and service coordination.
• Maintain records and prepare court-related documents.
• Certification as a Children’s Long Term Care Screener required after training.
• Document timely within 3 business days; maintain productivity.
• Maintain regular attendance; overtime as needed.
• Perform other duties.
