Job summary

Lutheran Social Services of WI and Upper MI is seeking a Care Coordinator to support families in the CLTS Waiver Program in Outagamie County, WI. You’ll be the primary contact for families, helping them access services to keep children safe and thriving.

This full‑time role offers a hybrid schedule with two days per week remote, requires a Bachelor's in Social Work or related field and at least one year of experience with children with disabilities, plus a valid driver's license and background

Qualifications

• Education: Bachelor’s degree in Social Work or a related Human Services field.
• Experience: Minimum 1 year working with children/youth with disabilities.
• Requirements: Valid driver’s license & reliable transportation; pass background and caregiver checks; strong documentation and communication skills; computer proficiency including EHRs.

Responsibilities

• Assess functional abilities and needs using required tools.
• Develop and implement individualized service plans with families and providers.
• Coordinate and authorize services per plan.
• Facilitate family-centered team meetings.
• Maintain accurate documentation and client records.
• Apply trauma-informed, evidence-based practices.
