Lutheran Social Services of Wisconsin & Upper Michigan is hiring a Care Coordinator for the CLTS Waiver Program in Walworth County, WI. This full-time role includes a hybrid work option and a focus on helping families access essential community services.

You’ll assess needs, develop service plans, coordinate across teams and agencies, and document thoroughly to meet regulatory timelines. Experience with children with disabilities and Spanish language skills are valued.

Qualifikationen

• Bachelor’s degree in Social Work or related field.
• Minimum 1 year of experience with children/youth with disabilities.
• Spanish bilingual preferred but not required.
• Proficient in computer systems and electronic health records.
• Strong interpersonal and organizational skills.
• Valid driver’s license and reliable transportation (MVR check required).
• Community-based with daily travel required.
• Flexibility to meet family needs, including crisis response.

• Assess children’s functional abilities using approved tools.
• Develop and implement individualized service plans with families and providers.
• Facilitate team meetings and coordinate services based on family-centered goals.
• Maintain accurate documentation and meet regulatory timelines.
• Collaborate with internal teams and external agencies to support families.
• Testify in legal proceedings when required.
• Communicate clearly with families, providers, and team members.
• Participate in staff development, training, and supervision.
