Job summary

Professional Services Group & Community Impact Programs in Madison, WI is hiring Case Managers for the Children's Long-Term Support (CLTS) Waiver Program. You will provide service coordination, assess needs, and connect families with counseling and community resources.

The role involves home visits, developing individualized service plans, and collaborating with families to address developmental, physical, or mental health needs.

Qualifications

• Bachelor's degree in sociology, social work, psychology or closely related field.
• One year of professional social work experience is required.
• Reliable transportation with valid driver's license and auto insurance.

Responsibilities

• Receive new client referrals and make initial contact.
• Conduct assessments of client needs through interviews and other investigations.
• Maintain an individualized service plan with clients and perform casework services.
• Maintain a caseload of clients with various social needs and safety concerns.
• Coordinate social, community and other services for clients.
• Complete in-person home visits to conduct assessments and maintain client contact.
