• Bachelor’s degree in social work or a related field.
• Appointment conditional on successful criminal and caregiver background checks.
• Must meet work-related transportation requirements for County business.

Responsibilities

• Promote safety, stability, and community inclusion for children and families; support in-home services when possible.
• Conduct strengths-based assessments to identify needs across domains and develop goal-focused service plans.
• Coordinate and monitor services with providers, schools, and community partners to achieve positive outcomes.
• Lead family-centered planning and team meetings to align services with goals and resources.
• Maintain documentation and reporting in accordance with CLTS and Medicaid waiver standards; ensure timely records.
