Family First Health in York, PA invites a Clinical Triage Registered Nurse to join our team. You will assess and triage patients, apply protocols, and guide care decisions in a fast-paced outpatient setting.

The role emphasizes patient safety, accurate documentation in the EHR, collaboration with physicians and care teams, and compassionate patient interactions while supporting work‑life balance.

Qualifications

• Graduate of an accredited nursing program.
• Current, unrestricted PA RN license and/or multi-state privilege, maintained in good standing.
• Minimum of two years of clinical experience as a Registered Nurse.
• Current BLS certification.
• Naloxone training/certificate of completion as required by FFH.

Responsibilities

• Provides clinical triage for patients presenting with acute, new, worsening, or time-sensitive symptoms, including walk-in patients and patients contacting the practice for clinical guidance.
• Utilizes clinical judgment, established protocols, standing orders, and evidence-based guidelines to determine the appropriate level and urgency of care.
• Identifies patients requiring immediate intervention and initiates appropriate emergency response procedures, including activation of emergency medical services when indicated.
• Reinforces provider-established plans of care and ensures patients understand follow-up instructions and parameters for seeking additional care.
• Documents patient assessments, interventions, communications, education, and disposition accurately and timely in the electronic health record.
