Job summary

WWC Physicians PC is seeking a Clinic Triage RN to assess patients, triage symptoms, and coordinate care in an outpatient setting. Responsibilities include phone and in-person triage, patient education, and collaboration with providers, all while maintaining HIPAA standards.

The role emphasizes timely, safe, and effective patient care with electronic health records documentation and adherence to nursing protocols. Benefits are offered for eligible staff.

Qualifications

• Current, unrestricted RN license in state of practice.
• ADN or BSN from an accredited nursing program.
• Experience in outpatient/ambulatory care settings preferred.
• Strong assessment, critical thinking and decision-making skills.
• Excellent verbal and written communication.

Responsibilities

• Perform telephone and in-person triage assessments to evaluate symptoms and health status.
• Determine urgency and recommend same-day or follow-up care or emergencies.
• Provide nursing guidance within scope of practice and educate patients.
• Coordinate appointments, referrals and tests with providers.
• Document triage assessments and communications in the EHR.
• Maintain HIPAA compliance and protect patient confidentiality.
• Support quality improvement and efficient clinic operations.
