Unity Care NW is seeking a skilled Triage RN to deliver centralized, protocol-driven clinical triage for patients from neonate to geriatric across the clinics. You will assess concerns, judge urgency, and route patients to appropriate levels of care, supporting timely and safe decision making to protect provider capacity.

Key duties include telephone triage, in-basket workflow management, and escalation when needed.

Qualifications

• RN license in Washington state is required.
• Triage experience in urgent care, primary care, or ED preferred.
• Ambulatory care experience is desirable.
• Familiarity with structured triage protocols and decision support tools.

Responsabilités

• Provide protocol-driven telephone triage for Unity Care NW patients.
• Assess symptoms, risk factors, and urgency; determine appropriate disposition.
• Route to RN visits, provider visits, or home care as needed; educate within scope.
• Collaborate with RNs, care managers, and providers to ensure smooth patient flow.
