Option Care Health, Inc. is seeking a Clinical Transition Specialist to educate patients, families and facility staff on home infusion services. The role focuses on placing patients at appropriate alternative sites, building relationships with referrals, and partnering with Account Managers to grow sales and achieve goals.

The position requires active RN or Pharmacist licensure and a minimum of two years in healthcare, with travel to care facilities and hospitals to support transitions.

Qualifications

• Licensed RN or Pharmacist in the practice state.
• At least 2 years in healthcare.
• Strong relationship-building with providers and care teams.
• Willingness to travel to facilities for education and sales activities.

Responsibilities

• Grow and maintain relationships with referrals sources to drive sales and patient starts.
• Educate hospitals, facilities, and patients about Option Care services and benefits.
• Assess patient suitability for care at alternative sites.
• Coordinate discharge planning and ensure timely patient transitions.
• Collaborate with Account Manager and Regional Sales Director on area plans.
• Provide hands-on education to patients and caregivers.
• Communicate with agency staff and medical teams throughout discharge.
• Collect and review pre-admission information and obtain sign-off.
• Assist with orders transfer via phone, fax, or e-prescribing.
• Support marketing and education programs for patients and referral sources.
• Maintain confidentiality and comply with legal guidelines.
• Travel to partner facilities, hospitals to support transitions.
