Job summary

Option Care Health is seeking a Clinical Transition Specialist to educate patients and their families about home infusion services and to ensure proper placement at alternative sites. You will collaborate with hospital staff, intake and case managers to coordinate care and drive timely discharges.

The role emphasizes building strong relationships with referral sources, achieving sales goals, and maintaining patient confidentiality in a regulated healthcare environment.

Qualifications

• Licensed RN or Pharmacist in the state of practice.
• Minimum 2 years in healthcare industry.
• Strong customer service and relationship-building skills.

Responsibilities

• Maintain and grow relationships with referrals sources to increase sales and patient starts.
• Serve as key contact to educate and assist referral sources about Option Care services.
• Collaborate with hospital staff and discharge planners to ensure smooth patient transitions to home or ATS.
• Coordinate pre-admission information and insurance payor details with patient registration.
