Job summary

Advocate Health’s Floyd Behavioral Health Services is seeking a full-time Behavioral Health O/P: Adult Partial Hospitalization Program (PHP) clinician. Work Monday through Friday, 8:00 a.m.

to 4:30 p.m., no major holidays or weekends, with a pay range of $28.55 to $42.85 per hour. Responsibilities include coordinating patient care, facilitating group therapy, and ensuring accurate medical records and discharge planning.

Qualifications

• Master’s Degree in counseling, social work, or related field from an accredited college.
• 3 years of related experience.
• Knowledge of Cognitive Behavioral Therapy and how to apply to a group setting.
• CPR and CPI certification within 90 days of employment.

Responsibilities

• Provision of Care: coord in patient care, facilitates group therapy, relapse prevention, coping skills, assess recovery environments, risk for relapse, and develop programming and therapeutic treatment.
• Documentation and Management of Medical Record: complete treatment plans, assessments, progress notes, weekly updates and group notes; discharge planning and referrals; audit charts.
