Responsibilities

• Coordinate patient care and facilitate group therapy sessions.
• Develop programming and therapeutic treatment for identified patients.
• Document group notes, Plan of Care, progress notes within 24 hours.
• Communicate treatment issues with nurses and physicians daily.
• Participate in discharge planning and treatment teams.
