Zusammenfassung

EventusWholeHealth is seeking a Clinician Support Specialist to assist the clinical care team, coordinate workflows, and support telehealth visits across facilities.

You will collaborate with Advanced Practice Clinicians and facility staff, handling patient information, documentation, and communication to ensure high-quality care and smooth operations. Regular travel to assigned facilities is required.

Aufgaben

• Prepare for daily clinical rounds with complete patient information, documentation, and schedules.
• Support clinicians in encounter documentation, data entry, and chart preparation.
• Coordinate and update patient schedules including admissions, discharges, and acute changes.
• Serve as liaison between clinicians, facility staff, patients, and families to support communication and quality care.
• Assist with triage needs and documentation as directed by the clinician.
• Organize and manage clinical documentation including lab results, diagnostic data, and patient records.
• Host and support telehealth visits for routine and acute care needs.
• Obtain and process patient consents and route documentation properly.
• Participate in facility meetings and communicate updates to the care team.
• Support discharge planning and ensure accurate patient census and records.
