Job summary

Midwest Health is seeking a Clinical Support RN to act as a primary clinical responder and escalation resource. You will provide real-time guidance, clinical decision-making, and care coordination to support communities and residents.

You will collaborate with Executive Directors and RCCs to manage complex clinical situations, communicate with physicians and families, and document all interactions in EMR systems. A compact/multistate LPN license is required.

Qualifications

• LPN license with compact/multistate recognition required.
• Experience guiding clinical decisions in real time and coordinating care across teams.
• Ability to document interactions clearly and maintain accurate medical records.

Responsibilities

• Provide real-time clinical guidance to staff and respond to calls and alerts.
• Manage and document complex clinical situations (falls, changes in condition, behaviors).
• Advise on appropriate level of care and resident interventions.
• Communicate with physicians, families, and staff.
• Document interactions in EMR or other software.
• Review, educate, and participate in QAPI programs.

Midwest Health is looking for a Clinical Support RN serves as a primary clinical responder and escalation resource, providing real-time guidance, clinical decision-making, and care coordination to assist communities and provide support as needed.
This person will work with the Executive Directors and RCCs (Resident Care Coordinator) to do the following:

• Perform real-time guidance as part of quality assurance and respond to incoming calls and alerts
• Manage and document complex clinical situations (falls, change in condition, behaviors)
• Advise on appropriate level of care and recommend for resident interventions
• Communicate with physicians, families, and staff
• Document interactions in EMR or other software
• Review, assess, educate, and provide guidance on senital events and participate in the QAPI program

This person have a compact/multistate Licensed Practical Nurse (LPN) license.
