Job summary

Dartmouth Health Keene Clinic is seeking a per diem Hospital Pharmacist to provide pharmaceutical care to patients in Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic. You will monitor drug therapies, interact with the healthcare team, educate patients and families, and ensure secure handling of medications within scope of practice.

Requirements include a Bachelor of Pharmacy degree (NH licensure required; Pharm.D preferred).

Qualifications

• Graduate of an accredited College of Pharmacy; NH Pharmacist licensure required.
• Pharm.D preferred; hospital pharmacist experience preferred but training possible.
• Current NH Pharmacist licensure is required and must be active.

Responsibilities

• Provide pharmaceutical care to patients and monitor drug therapies as part of the health care team.
• Review physician orders for safety, appropriateness, and policy compliance; report issues to practitioners.
• Enter medication orders, allergies and pertinent information into the patient record; document changes as needed.

Job description

Responsible for the provision of pharmaceutical care to the patients of Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic through monitoring of drug therapies, interaction with other members of the healthcare team and the education of patients and their family members. Receives, handles, stores, and/or disposes of medications according to policy and/or standards within one's scope of service and/or practice, taking care to limit access and secure medications throughout these processes. Reviews physician orders for safety, appropriateness and compliance with all relevant policies and procedures. Contacts practitioners and other staff when issues or problems are discovered. Able to resolve problems discovered during this review. Documents changes made in the appropriate record. Enters medication orders, allergies and any other pertinent clinical or other information into the appropriate patient medical record.

Graduate of an accredited College of Pharmacy. A Bachelor's degree from an accredited school of pharmacy is required. Pharm.D (preferred). Experience as a hospital pharmacist is preferred but will train motivated candidates. Must have current State of NH Pharmacist licensure.
