Job summary

Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic seeks a pharmacist to provide pharmaceutical care, monitor therapies, and educate patients and families. The role involves reviewing orders, coordinating with the health care team, and securely handling medications.

Applicants must Graduate from an accredited College of Pharmacy with a Bachelor's degree; Pharm.D is preferred. NH pharmacist licensure is required; hospital experience is desired but training is available.

Responsibilities

• Review physician orders for safety and compliance with policies and procedures; contact practitioners when issues arise and resolve problems.
• Enter medication orders, allergies and pertinent clinical information into the patient medical record.

Job description

Responsible for the provision of pharmaceutical care to the patients of Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic through monitoring of drug therapies, interaction with other members of the healthcare team and the education of patients and their family members. Receives, handles, stores, and/or disposes of medications according to policy and/or standards within one's scope of service and/or practice, taking care to limit access and secure medications throughout these processes. Reviews physician orders for safety, appropriateness and compliance with all relevant policies and procedures. Contacts practitioners and other staff when issues or problems are discovered. Able to resolve problems discovered during this review. Documents changes made in the appropriate record. Enters medication orders, allergies and any other pertinent clinical or other information into the appropriate patient medical record.

Graduate of an accredited College of Pharmacy. A Bachelor's degree from an accredited school of pharmacy is required. Pharm.D (preferred). Experience as a hospital pharmacist is preferred but will train motivated candidates. Must have current State of NH Pharmacist licensure.
