Job summary

Dartmouth Health, Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic, seeks a pharmacist to provide pharmaceutical care to patients, monitor therapies, educate patients and families, and ensure safe handling of medications. Responsibilities include reviewing orders, coordinating with clinicians, and documenting all changes in the patient record.

Graduates of an accredited College of Pharmacy are encouraged to apply; a Bachelor's degree is required and Pharm.D is preferred.

Qualifications

• Graduate of an accredited College of Pharmacy.
• A Bachelor's degree from an accredited school of pharmacy is required.
• Pharm.D (preferred).
• Experience as a hospital pharmacist is preferred but will train motivated candidates.
• Must have current State of NH Pharmacist licensure.

Responsibilities

• Provide pharmaceutical care to patients and monitor drug therapies; educate patients and their families.
• Review physician orders for safety, appropriateness and policy compliance; resolve issues with practitioners and staff.
• Enter medication orders, allergies and other pertinent information into the patient medical record and document changes.
