Responsibilities

  • Review physician orders for safety, appropriateness and policy compliance.
  • Communicate issues to practitioners and staff and resolve problems identified.
  • Document changes in the patient record and enter orders and allergies into the medical record.

Job description

Responsible for the provision of pharmaceutical care to the patients of Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic through monitoring of drug therapies, interaction with other members of the healthcare team and the education of patients and their family members. Receives, handles, stores, and/or disposes of medications according to policy and/or standards within one's scope of service and/or practice, taking care to limit access and secure medications throughout these processes. Reviews physician orders for safety, appropriateness and compliance with all relevant policies and procedures. Contacts practitioners and other staff when issues or problems are discovered. Able to resolve problems discovered during this review. Documents changes made in the appropriate record. Enters medication orders, allergies and any other pertinent clinical or other information into the appropriate patient medical record.

Compensation

Minimum Pay: $48.71

Maximum Pay: $65.77
