Job summary

Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic seeks a hospital pharmacist to provide pharmaceutical care, monitor therapies, interact with the care team and educate patients and their families. Responsibilities include handling medications according to policy and documenting changes in the medical record.

Required: graduate of an accredited College of Pharmacy, a Bachelor's in Pharmacy, NH pharmacist licensure; Pharm.D preferred.

Qualifications

  • Graduate of an accredited College of Pharmacy.
  • A Bachelor's degree from an accredited school of pharmacy is required.
  • Pharm.D preferred.
  • Must have current State of NH Pharmacist licensure.
  • Hospital pharmacist experience preferred; will train motivated candidates.

Responsibilities

  • Provide pharmaceutical care to patients and educate patients and families.
  • Review physician orders for safety and appropriateness.
  • Enter medication orders, allergies and pertinent information into the patient record.
  • Monitor drug therapies and communicate issues to the healthcare team.

Education

Bachelor's degree in Pharmacy
Pharm.D preferred
NH Pharmacist license

Job description

Responsible for the provision of pharmaceutical care to the patients of Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic through monitoring of drug therapies, interaction with other members of the healthcare team and the education of patients and their family members.Receives, handles, stores, and/or disposes of medications according to policy and/or standards within one’s scope of service and/or practice, taking care to limit access and secure medications throughout these processes.Reviews physician orders for safety, appropriateness and compliance with all relevant policies and procedures. Contacts practitioners and other staff when issues or problems are discovered. Able to resolve problems discovered during this review. Documents changes made in the appropriate record.Enters medication orders, allergies and any other pertinent clinical or other information into the appropriate patient medical record.

Graduate of an accredited College of Pharmacy. A Bachelor's degree from an accredited school of pharmacy is required. Pharm.D(preferred). Experience as a hospital pharmacist is preferred but will train motivated candidates. Must have current State of NH Pharmacist licensure.
