Job summary

Cheshire Medical Center in Keene, NH, seeks a pharmacist to provide pharmaceutical care, review orders for safety and policy compliance, and educate patients and families. You will handle medications securely, coordinate with clinicians, and document changes in the patient record.

Hospital experience is preferred but training is available for motivated candidates. Ideal candidates hold a state license to practice in New Hampshire; a PharmD is preferred.

Qualifications

• Graduate of an accredited College of Pharmacy.
• A Bachelor's degree from an accredited school of pharmacy is required.
• Pharm.D (preferred).
• Experience as a hospital pharmacist is preferred but will train motivated candidates.
• Must have current State of NH Pharmacist licensure.

Responsibilities

• Responsible for the provision of pharmaceutical care to patients by monitoring drug therapies and educating patients and families.
• Review physician orders for safety and compliance with policies; contact practitioners when issues arise.
• Enter medication orders, allergies, and pertinent information into the patient record; secure medications.

Job description

Responsible for the provision of pharmaceutical care to the patients of Cheshire Medical Center/Dartmouth-Hitchcock Keene Clinic through monitoring of drug therapies, interaction with other members of the healthcare team and the education of patients and their family members. Receives, handles, stores, and/or disposes of medications according to policy and/or standards within one's scope of service and/or practice, taking care to limit access and secure medications throughout these processes. Reviews physician orders for safety, appropriateness and compliance with all relevant policies and procedures. Contacts practitioners and other staff when issues or problems are discovered. Able to resolve problems discovered during this review. Documents changes made in the appropriate record. Enters medication orders, allergies and any other pertinent clinical or other information into the appropriate patient medical record.

Compensation

Minimum Pay: $48.71
