Qualifications

• Master's degree in psychology, counseling or related clinical field.
• Valid NYS MSW license.
• Two years of full-time case management or social work experience.
• Experience in mental health, PTSD, depression, anxiety or substance abuse.

Responsibilities

• Provide clinical assessment and care plan management for WTCHP members.
• Coordinate medical and psychosocial services with internal and external teams.
• Perform referrals, authorizations and education about program benefits.
• Document in EMR and internal systems; collaborate across departments.
• Travel between Westbury and Commack clinics; some evenings and Saturdays.
