Job summary

The Research Foundation for The State University of New York at Stony Brook is seeking a Social Worker to join the Department of Medicine - WTCHP in Commack/Westbury. You will provide clinical assessment, develop care plans, and coordinate medical and psychosocial services for WTCHP members.

Ideal candidates hold a master's degree and NY MSW license, with 2 years of case management or social work experience, and strong interpersonal and written communication skills.

Qualifications

• Master's degree in psychology, counseling or related clinical field and NY MSW license.
• Two years of full-time case management or social work experience.
• Mental health experience, especially PTSD, depression, anxiety or substance abuse.

Responsibilities

• Provide clinical assessment by reviewing self-report measures or conducting diagnostic interviews.
• Care/case management for WTCHP members with complex medical health conditions, mental health conditions, and/or cancer.
• Educate patients about WTCHP benefits and limitations.
• Perform appropriate referral and disposition planning.
• Liaise between members, our providers, and external providers treating members.
• Documentation in electronic medical records and internal programs.
