Job summary

Stony Brook University in Commack/Westbury seeks a Social Worker to coordinate comprehensive care for WTCHP members and provide clinical assessment. The role requires NYS MSW license, a Master's in a related field, and 2+ years of case management or social work experience.

Qualifications

• Master's degree in psychology, counseling or related clinical field.
• Valid NYS MSW license.
• Two years full-time case management or social work experience.
• Mental health experience, PTDS, depression, anxiety or substance abuse.

Responsibilities

• Provide clinical assessment and facilitate care plan management.
• Act as liaison between clinical providers, psychosocial services and staff.
• Coordinate care for WTCHP members with complex conditions.
• Educate patients about program benefits and limitations.
• Prepare authorizations for services and higher-level reviews.
• Collaborate with internal departments to identify member needs.
• Document in EMR and internal databases.
