Qualificações

• MSW from an accredited Social Work Education School.

• Strong psychosocial assessment skills.
• Understanding of discharge planning and barriers to long-length stay.

Responsabilidades

• Perform psychosocial assessments for selected patients.
• Attend rounds and coordinate with the medical team.
• Collaborate with community agencies for post-discharge needs.
• Coordinate discharge planning and insurance verification.
• Assist with Medicaid applications and benefits where needed.
• Supervise social work students and participate in program evaluation.
