Mansioni

• Psychosocial assessment of selected cases.
• Effectuate discharge planning.
• Attend rounds and coordinate with medical team.
• Work with community agencies for post-discharge needs.
• Coordinate with SB Outpatient division for continuum of care.
• Documentation of Social Work interventions.
• Assist patients with Medicaid and insurance reviews as needed.
• Coordinate and review with insurers for authorizations.
