• Attend rounds and coordinate with medical team.
• Coordinate with community agencies for post-discharge needs.
• Work with SB Outpatient division for continuum of care.
• Documentation of Social Work interventions.
• Assist with discharge planning and insurance reviews as needed.
• Supervise social work students and participate in program development.
• Engage in performance improvement activities related to palliative care.
