Job summary

Corewell Health in Grand Rapids seeks a Part-time Social Worker (MSW) to coordinate care management and behavioral health interventions for patients with complex needs. You will assess, plan, monitor, and provide therapeutic interventions while collaborating with patients and families to promote healthier outcomes.

Responsibilities include crisis response, care conferences participation, and partnering with community resources to support patients across the continuum of care.

Qualifications

• Master of Social Work with licensure or eligibility required.
• MSW with State of Michigan requirements, excludes VNA and Rural Health Clinics, upon hire.
• Master Social Work LTD qualification as stated by employer.

Responsibilities

• Coordinate care management and behavioral health interventions.
• Develop strategies for cost-effective, quality care outcomes for patients with complex needs.
• Assess, plan, monitor, and provide therapeutic interventions as appropriate.
• Collaborate with patient and family to facilitate understanding and necessary lifestyle changes.
