Responsibilities

• Assist patients and families with psychosocial needs and complex situations.
• Coordinate alternate sites of care and discharge planning.
• Address abuse/neglect and connect to appropriate resources.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources and services.
• Participate in department, regional, or statewide committees as assigned.
• Develop and maintain awareness of federal and state regulations related to the role.
• Document assessments and plans in the EMR reflecting current condition and transitions.
