• Training and competency in casework principles, including crisis intervention and transition planning.

Aufgaben

• Provide social work intervention related to hospitalization and discharge planning, including crisis management and ethical/legal concerns.
• Collaborate with health care team to transition patient to appropriate level of care and advocate for patient self-determination.
• Assist with legal/clinical documentation, including competency determinations and guardianship processes as needed.
• Coordinate with care management team to provide information on admission, stay, discharge criteria, and regulatory standards.
