Job summary

Tenet Healthcare's DMC Sinai-Grace Hospital is seeking a Clinical Social Worker Case Manager to facilitate patient transitions, coordinate care, and support discharge planning. The role includes psycho-social assessments, collaboration with care teams, and patient education to ensure optimal utilization of resources.

Requirements include a Master's in Social Work and Michigan MSW licensure, with two years of acute hospital experience preferred. ACM credential is preferred.

Qualifications

• Master's degree in Social Work from an accredited program.
• Current license as a Licensed MSW in Michigan, or license with 3-year window for full licensure.
• Two years of acute hospital experience preferred.
• ACM (Accredited Case Manager) credential is preferred.

Responsibilities

• Complete comprehensive psycho-social assessments and transition planning.
• Coordinate care and facilitate transition plans for patients to support timely discharge.
• Collaborate with physicians, nurses, and other staff to ensure plan of care aligns with patient needs.
• Provide education to patients and families regarding resources and benefits available.
