Job summary

Oak Street Health in Richmond, VA is seeking a Medical Scribe to support primary care providers with real-time clinical documentation. Scribes observe in the exam room, document encounters, assign CPT and ICD-10 codes, and help prepare After Visit Summaries while aligning with our value-based care model.

The role emphasizes accuracy, fast typing (70+ wpm preferred), in-person clinic work, full-time hours about 40-45 per week, HIPAA compliance, and willingness to commit 1 year or more.

Qualifications

• Knowledge of medical terminology and common medications.
• Prior clinical experience, including shadowing and/or volunteering.
• Prior scribe or transcription experience is preferred but not required.

Responsibilities

• Document patient encounters in a structured note including HPI, assessment, plan, and exam.
• Assign CPT and ICD-10 codes and prepare After Visit Summaries.
• Support clinical documentation improvement and review records.
