Job summary

Mosaic Community Health in Bend, Oregon is seeking a Referral Health Information Technician to ensure timely delivery of medical records and processing of referrals. You will support ROI processing, site coordination, and education of care teams on HIT/Referrals workflows, helping maintain accurate patient charts and efficient referral

Qualifications

• One year of experience in a medical office setting.
• Referral coordination and health record management preferred.
• Experience working with low-income populations.
• Experience working on computers; typing at least 40 WPM.
• Knowledge of Microsoft Office Software products and medical terminology preferred.
• Fluency in Spanish preferred.

Responsibilities

• Daily am/pm distribution of documents and medical records; coordinate faxes for timely distribution.
• Manage interoffice mail to central HIT/Referrals office and log documents between Mosaic sites.
• Ensure documents meet signature requirements and contain 3 patient identifiers.
• Maintain clinic fax number; report incorrect documents and fax corrections to Central HIT.
• Process site ROIs to gather information before requesting records.
• Create an Epic encounter and document ROI requests.
• Educate staff on HIT/Referrals processes and troubleshoot related issues.
• Instruct staff on documents approved to be scanned into patient charts.
• Process site-specific urgent referrals and assist with system-wide referrals.
• Navigate the CIMS website to check referral status and share with care teams.
• Track referrals in collaboration with the centralized referrals team.
• Act as liaison for site and central Referrals to clarify issues and share referral information.
