NorthLakes Community Clinic is seeking a Clinical Quality Improvement Manager to lead quality improvement initiatives, support patient-centered medical home (PCMH) accreditation, and guide teams in evidence-based, lifelong care. The role emphasizes collaboration with RNs, community health workers, and other staff to implement QI projects.

The position requires nursing education, 3–4 years clinical experience, and at least 1 year in quality improvement, with proficiency in EHR and data analysis.

Qualifications

• Bachelor's degree in nursing or ADN required.
• 3–4 years of clinical experience or equivalent training (RN, MA, or similar).
• At least 1 year of quality improvement and practice transformation experience.
• Experience with EHR systems, data analysis, and population health management tools.

Responsabilités

• Guides implementation of quality improvement and PCMH efforts.
• Ensures QI/QA assessments are conducted and outcomes monitored.
• Develops, implements, and updates QI/QA operating procedures.
• Coordinates NCQA PCMH readiness, reporting for accreditation.
• Develops care management protocols for specific patient populations.
• Analyzes quality metrics and prepares reports to support improvement.
