Job summary

NorthLakes Community Clinic is seeking a Clinical Quality Improvement Manager to guide quality initiatives and PCMH implementation across teams. You will monitor QA metrics, develop procedures, and collaborate with nurses and staff to improve patient outcomes and care management processes.

You will oversee training, EHR documentation quality, and reporting to meet accreditation standards while advancing value-based care in our service area.

Qualifications

• Bachelor’s Degree (B.A. or B.S.N) or ADN; RN preferred
• 3-4 years clinical experience in a similar role; experience in quality improvement and practice transformation
• Experience with EHR, data analysis/population health management programs, and Excel is required (or preferred)

Responsibilities

• Guides implementation of quality improvement and PCMH efforts.
• Ensures QI/QA assessments are conducted and outcomes monitored.
• Responsible for development, implementation and updating of QI/QA operating procedures.
• Partners with teams on successful implementation of NCQA PCMH standards and coordinates reports to maintain PCMH accreditation.
• Develops processes for patient outreach to close care gaps and improve outcomes.
• Collaborates in development and training to support consistent documentation in the EHR for quality measures.
• Analyzes quality metrics, gathers information, prepares reports and summarizes findings to support quality improvement.
• Maintains current quality assurance standards, regulations and requirements to support the organization in reaching benchmarks and high quality care.
• Collaborates with Directors and Officers to develop protocols, policies and procedures.
• Develops and delivers training to ensure safe and evidence-based care and promote quality improvement.
• Works with nurses, community health workers and staff to develop care management protocols for patient populations.
• Partners with teams to support team-based care models and other innovative quality programming.
• Assists with related activities for committees and meetings; manages quality staff to meet deliverables.
• Prioritizes concurrent tasks and timelines; identifies obstacles and proposes timely solutions.
