Job summary

NorthLakes Community Clinic in Ashland, WI seeks a Clinical Quality Improvement Manager to lead quality initiatives, support PCMH accreditation, and promote evidence-based, patient-centered care across teams.

The role requires RN- preferred and CCE certification, 3–4 years clinical experience plus QA experience, and collaboration with directors to implement protocols. Partial remote work is available.

Qualifications

• Minimum education includes a Bachelor's in nursing or related field, or an ADN with equivalent experience.
• 3–4 years of clinical experience plus at least 1 year in quality improvement and practice transformation.
• Experience with EHR, data analysis, population health management, and Excel required.

Responsibilities

• Guides implementation of quality improvement and PCMH efforts.
• Ensures QI/QA assessments are conducted and outcomes monitored.
• Develops and updates QI/QA operating procedures.
• Coordinates reports to maintain PCMH accreditation.
• Develops patient outreach to close care gaps and improve outcomes.
• Delivers training to support safe, evidence-based care.
