Reliant Rehabilitation is seeking a Physical Therapist to deliver comprehensive therapy services in skilled nursing facilities and LTACH partnerships. You will assess patients, develop and adjust treatment plans, and collaborate with a multidisciplinary team across facilities.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Active physical therapist license in the state.
• Proficient in clinical documentation and basic computer skills.

Responsibilities

• Provide physical therapy services and develop treatment plans.
• Direct and supervise PTAs and Rehab Techs as appropriate.
• Ensure compliance with OBRA, state practice acts, and Reliant policies.
• Maintain infection control and patient safety protocols.
